In Khan Tusion's Hellfire Sex IV, it's the same as the others in this series. Sandra Romaine, Shy Love, and Melissa Lauren torture various porn stars until they're tenderized enough to get ass-slammed by the usual super rough JM male performers. Sandra's ass-milk scene was pretty wild, I tell ya that much. The rest is just a blur of spitting and slapping, as with most scenes in Hellfire Sex.
